1. What volunteers do. Dignity Discharge: sort donated retail items and pack take-home kits; volunteers never meet patients or see patient information. Service hours are recorded; we can't promise a school will accept them.
2. Volunteer promises. Follow the adult leader and the safety rules, be respectful, keep partner information private, don't post others' names or faces without permission, tell an adult right away if hurt or unsafe. Volunteers are not employees and are not paid.
3. Parent/guardian consent. I am the parent or legal guardian and allow my child to take part in the selected activities. I will not put health details on this form; I will tell the adult leader in person.
4. Assumption of risk and release. Volunteering has ordinary risks (lifting, cuts, slips, food handling, travel). To the extent Texas law allows, I release the programs, their organizers, partner sites and their volunteers from claims for ordinary negligence. This does not cover gross negligence or intentional misconduct.
5. Emergency. If I can't be reached, the adult leader may call 911 and get emergency care for my child.
6. Photo release (optional, separate box below). Photos/video of my child volunteering may be used on program sites, social media, school projects and competition entries, unpaid, with no last name shown. I can withdraw this for future uses by email.
7. Privacy. We keep only the fields on this form plus this signature record, use them only to coordinate volunteering, and never sell them. A parent can ask to see or delete the record.
8. Electronic signature. Typing my full legal name, signing, and checking the boxes is my electronic signature (Texas UETA). The date and time are recorded by the server.
